On the 01 April 2024, Grove Park Surgery, Wellesley Road Practice and Chiswick Family Doctors Practice have now merged to form Chiswick Medical Practice.

Our main site will be Grove Park Site at Burlington Lane and we will continue to deliver services at Wellesley Road site (previously Wellesley Road Practice) and Chiswick Health Centre Site (previously Chiswick Family Doctors Practice).
